• Our practitioners provide camp physical service to children under the age of 18 years old

  • Insurance is not accepted for camp physical and payment is due at the time of visit

  • Be prepared to discuss your child's past medical history, current treatments, conditions, allergies or medications

  • Tampa Urgent Care can not sign off on high-risk medical physicals. We recommend you see your primary care physician for these specific physicals

  • Bring a copy of your camp physical form (if required by your camp/activity) for the provider to complete

    • Boy & Girl Scouts: Parents must complete the informed consent and general information/ health histor​y before visit

  • Bring a copy of your latest immunizations for review​​

  • Bring a copy of your child's eye glasses or contact lens prescription, if they wear them

  • A camp physical should not be replacement for an a annual wellness exam 


Camp physicals are a great way to ensure it is safe for you to participate scouts, day or sport camp.  Tampa Urgent Care's fully trained staff will review your medical history and complete a physical examination. Our goal is to find out about and deal with any health problems that might interfere with your participation for your child's summer activity. ​

  • No appointment is necessary, so visit us anytime at your convenience 

  • Be prepared to discuss you past medical history, current treatments, conditions, allergies and/or medications

  • Completion of camp physical form (if required by your camp/activity).  Additionally, we will provide summary of the physical results and proof of payment
